Dependent Lung Pulmonary Artery Hypoplasia as a Cause of Hypoxia during One-lung Ventilation

چکیده

Hypoxemia remains a well-described occurrence during one-lung ventilation.1 We recently observed unexpected refractory hypoxemia ventilation in previously healthy nonsmoking 30-yr-old female undergoing left thoracotomy the right lateral decubitus position to facilitate complex descending aorta subclavian/carotid artery bypass grafting treat upper limb and cerebral ischemia resulting from Takayasu’s arteritis. Despite endotracheal suctioning, reconfirmation of optimal bronchial blocker positioning, optimizing fraction inspired oxygen, using both dependent lung positive end-expiratory pressure nondependent continuous airway pressure,2 Pao2 remained ~50 mmHg. No intracardiac shunt was seen on transesophageal echocardiography. Only intermittent two-lung temporarily corrected hypoxemia. A postoperative computed tomography angiogram three-dimensional reconstruction (panel A) performed evaluate vascular showed hypoplastic pulmonary (10 mm diameter; panel B) compared normal (19 C). speculate that this hypoplasia resulted enhanced shunting nondependent, nonventilated lung; is unreported potential cause for Although estimated occur only 1:200,000 population, consideration should be given reviewing routinely acquired preoperative scan assess size differential as rare risk factor ventilation. If present, anticipation an increased prompt rapid escalation adjunct therapies (i.e., pressure; ventilation, with conversion thoracoscopic technique open thoracotomy). Extracorporeal membrane oxygenation also option. The authors declare no competing interests.

Pulmonary Artery Agenesis and Lung Hypoplasia

Unilateral pulmonary artery agenesis with lung hypoplasia is very rare and only a few cases have been reported in the literature. Some patients with hypoplasia may suffer from recurrent hemoptysis dyspnea, and lung infections. Other patients are asymptomatic. Chest X-Ray reveal a reduction in the volume of hemithorax, an elevation in the diaphragm, reduced intercostals spaces and mediastinal sh...

Independent Lung Ventilation and One Lung Ventilation

The application of the independent lung ventilation (ILV) in adults has been described by Carlon and co-workers in 1978 and subsequently by other investigators. In 1983 Hedenstierna and coworkers using a bilumen tube suitable for prolonged intubation, and two Servo Ventilator 900 B (Siemens) adapted for synchronization, treated a wide range of clinical conditions. Following this experience, the...

The use of air in the inspired gas mixture during two-lung ventilation delays lung collapse during one-lung ventilation.

BACKGROUND Collapse of the ipsilateral lung facilitates surgical exposure during thoracic procedures. The use of different gas mixtures during two-lung ventilation (2LV) may improve or impede surgical conditions during subsequent one-lung ventilation (OLV) by increasing or delaying lung collapse. We investigated the effects of three different gas mixtures during 2LV on lung collapse and oxygena...

Pressure-controlled ventilation and intrabronchial pressure during one-lung ventilation.

BACKGROUND Pressure-controlled ventilation (PCV) has been suggested to reduce peak airway pressure (P(peak)) and intrapulmonary shunt during one-lung ventilation (OLV) when compared with volume-controlled ventilation (VCV). At the same tidal volume (V(T)), the apparent difference in P(peak) is mainly related to the presence of a double-lumen tracheal tube. We tested the hypothesis that the decr...

Exhaled volatile organic compounds as lung cancer biomarkers during one-lung ventilation

In this study, single-lung ventilation was used to detect differences in the volatile organic compound (VOCs) profiles between lung tissues in healthy and affected lungs. In addition, changes that occurred after lung cancer resection in both the VOCs profiles of exhaled breath from ipsilateral and contralateral lungs and the VOCs profiles of exhaled breath and blood sample headspaces were also ...
